Having seen the removal of Combe from the Distinguished Service Order page (where he was listed with those who were awarded the DSO and three bars), I did a little digging on the London Gazette site - relevant pages have now been added to the reference list. I couldn't find the announcement of his DSO, but I did find his appointment as a CB, grant of approval to wear the Legion of Honour (Officer)(US) and Notice Under the Trustees Act upon his death. The accompanying post-nominals on these entries indicates that he had previously been awarded the DSO but doesn't refer to any bars. Given that it was the LG's custom to refer to bars where appropriate, I think it safe to say he was only awarded the DSO once, as reflected in this page's lead section, not three times.
